What can I expect on my first visit?
On your first visit to our office, before any therapy is undertaken, we will provide you with the necessary examination. Following your examination, we will discuss the periodontal treatment that will be best, the options for treatment, fees for the treatment, and the risks to your oral health if appropriate treatment is not completed. With this information, you will be able to make an informed decision regarding your treatment.
What do I need to bring on my first visit?
We will mail you a new patient packet prior to your appointment. Please complete these forms and bring them with you on the day of your appointment. If you are unable to complete the patient information form prior to your appointment time, please arrive fifteen minutes early to do so. Please bring your referral slip and any available x-rays from your referring dentist. If you have dental benefits, please bring that information with you, too.
Health history:
In our office we think of periodontal care as part of your overall health program—so we must consider your total health. Please take special care filling out our health forms. Please bring a list of your current medications with you to your first appointment. Please inform us regarding any medication changes you experience or if you begin a treatment program with your physician. Of course, your health history is confidential, and may not be released without your consent.
